State revokes, suspends licenses, certifications, registrations of health care providers (11-13-2025)

OLYMPIA -- The Washington State Department of Health has revoked or suspended the licenses, certifications, or registrations of the following health care providers in our state. The department has also immediately suspended the credentials of people who have been prohibited from practicing in other states.
The department’s Health Systems Quality Assurance Division works with boards, commissions and advisory committees to set licensing standards for more than 80 health care professions (e.g., dentists, nurses, counselors). Information about disciplinary action taken against medical doctors and physician assistants can be found on the Washington Medical Commission (WMC) website. Questions about WMC disciplinary actions can be sent to media@wmc.wa.gov.
Information about health care providers is on the agency’s website. Click on “Health Care Provider Lookup” under the “Find it Fast” section of the Department of Health website (doh.wa.gov). The site includes information about a health care provider’s license status, the expiration and renewal date of their credential, disciplinary actions and copies of legal documents issued after July 1998. This information is also available by calling 360-236-4700. Consumers who think a health care provider acted unprofessionally are also encouraged to call and report their complaint.
Clark County
In October 2025 the Department of Health and Wendy Amelia Kurinzi (NA61509754) entered an agreed order indefinitely suspending Kurinzi’s registered nursing assistant credential. In July 2024, the Department of Social and Health Services determined Kurinzi abused a vulnerable adult and placed her on its long-term care abuse and neglect registry. Placement on the registry prohibits Kurinzi from working with or having unsupervised access to vulnerable adults.
In October 2025 the Department of Health and Constance M. Morris (NA00101484) entered an agreed order in which Morris surrendered her registered nursing assistant credential. Morris is prohibited from resuming practice in Washington State, including temporary, emergency or volunteer practice. In April 2025, the Department of Social and Health Services determined Morris physically abused a vulnerable adult and placed her on its long-term care abuse and neglect registry effective February 2025. Placement on the registry prohibits Morris from working with or having unsupervised access to vulnerable adults.
Chelan County
In September 2025 the Department of Health summarily suspended Jose Antonio Vasquez’s (NC60897228) certified nursing assistant credential, pending further disciplinary action. In January 2025, the Department of Social and Health Services determined Vasquez financially exploited a vulnerable adult and placed him on its long-term care abuse and neglect registry. Placement on the registry prohibits Vasquez from working with or having unsupervised access to vulnerable adults.
Cowlitz County
In October 2025 the Department of Health and Jorge A. Brito (NA61419591) entered an agreed order indefinitely suspending Brito’s registered nursing assistant credential. In March and May 2024, Brito failed to safely assist with patient transfers, resulting in serious injuries.
In October 2025 the Department of Health and Amanda Kay McCoy (NC10097306) entered an agreed order indefinitely suspending McCoy’s certified nursing assistant credential. In February 2024, McCoy acknowledged the need for substance use disorder treatment.
In October 2025 the Department of Health summarily suspended Lisa K. Robert’s (NA60140397) registered nursing assistant credential, pending further disciplinary action. In May 2025, the Department of Social and Health Services determined Robert abused a vulnerable adult and placed her on its long-term care abuse and neglect registry. Placement on the registry prohibits Robert from working with or having unsupervised access to vulnerable adults.
Pierce County
In October 2025 the Department of Health summarily suspended Emily Maria Cook’s (PC61675224) medical assistant-phlebotomist credential, pending further disciplinary action. In May 2025, the Department of Social and Health Services determined Cook abused a vulnerable adult and placed her on its long-term care abuse and neglect registry. Placement on the registry prohibits Cook from working with or having unsupervised access to vulnerable adults.
Snohomish County
In October 2025 the Department of Health indefinitely suspended Gladworth Ochoa Flynn’s (HM60966457) home care aide credential. In December 2024, Flynn admitted to accepting $24,737.29 from a patient. In June 2025, she waived her right to a hearing and submitted a written statement, which the disciplinary authority considered when deciding the case.
Spokane County
In September 2025 the Department of Health summarily suspended Leslie Carol Boettcher’s (NA60598794) registered nursing assistant credential, pending further disciplinary action. In June 2025, the Department of Social and Health Services determined Boettcher mentally abused a vulnerable adult and placed her on its long-term care abuse and neglect registry effective March 2025. Placement on the registry prohibits Boettcher from working with or having unsupervised access to vulnerable adults.
In October 2025 the Pharmacy Quality Assurance Commission and Jennifer Lee Crownover (VA60113243) entered an agreed order which indefinitely suspended Crownover’s pharmacy technician credential. In March 2024, Crownover tested positive for one or more controlled substances and marijuana during an employer-administered drug test and was terminated from employment.
Thurston County
In September 2025 the Department of Health indefinitely suspended Sasha Veronica Brown’s (NA61318810) registered nursing assistant credential. In May 2025, the Department of Social and Health Services determined Brown financially exploited a vulnerable adult and placed her on its long-term care abuse and neglect registry. Placement on the registry prohibits Brown from working with or having unsupervised access to vulnerable adults. Brown has not responded to the department’s request for information.
Whatcom County
In October 2025 the Pharmacy Quality Assurance Commission indefinitely suspended Tucker Lawrence Dufrane’s (VB61258285, VA61426703) pharmacy assistant and pharmacy technician credentials. In March 2024, Dufrane admitted to being under the influence of alcohol while at work. He has not responded to the Commission’s request for information.
In September 2025 the Department of Health suspended Lazarus Augustine Wentz’s (CG60815995, MC60851085) agency affiliated counselor and associate mental health counselor credentials. Between November 2023 and February 2024, Wentz contacted a former minor client’s parent and two former clients after suspension and also resided on clients’ properties. Wentz may not petition for reinstatement of either credential for at least five years. He has not responded to the department’s requests for information.
